Management of Personal Information
This document outlines HKPP's policy for handling clients' personal information, in compliance with Hong Kong's Personal Data (Privacy) Ordinance (Cap. 486) (PDPO).
Collection of Client Information
Client files are securely stored in an electronic document management system, accessible only to authorized personnel in accordance with HKPP's policies and procedures. Each file may include personal details such as name, address, contact information, medical history, and other relevant data collected during the provision of psychological services.
Methods of Collecting Personal Information
Personal information is collected through various means, including:

Direct provision by clients via forms, emails, or interactions with HKPP staff.

Information shared by other health practitioners through referrals, correspondence, or medical reports.

Consequences of Withholding Personal Information
If a client chooses not to provide certain personal information as outlined in this policy, HKPP may be unable to offer the full scope of psychological services. Clients may request anonymity or the use of a pseudonym, except where impractical for HKPP or when legally required to identify individuals.
Purpose of Collecting Personal Information
The primary purpose of collecting personal information is to deliver effective psychological services, including assessment, diagnosis, and treatment. This information documents session details and enables the psychologist to provide informed care. Personal data may also be used for marketing purposes, with client consent.
Disclosure of Personal Information
Client information remains confidential, except under circumstances where disclosure is:

Necessary to prevent a serious threat to life, health, or safety.

Authorized by the client for:

Providing reports to other professionals (e.g., GPs, lawyers).

Discussing information with designated individuals (e.g., family members, employers, health providers).

HKPP does not disclose personal information to overseas recipients without client consent, unless legally obligated. Personal data will not be sold, rented, or used for unrelated purposes.
In the event of unauthorized access, disclosure, or loss of personal information, HKPP will implement its data breach response plan to mitigate potential harm.
Access and Correction of Personal Information
Clients may request access to or correction of their personal information at any time. Requests should be directed to admin@hkpsychologypractice.com. HKPP will respond in writing within 30 days and may arrange an appointment for clarification if necessary.
